Critical results: closing the loop from notification to action

Assign ownership, acknowledgment and follow-up when sending an alert is insufficient.

← All Healthcare articles

The risk continues after reporting

A result can be transmitted without anyone taking responsibility for follow-up. Joint Commission describes a closed loop as sending, receiving, acknowledging and acting on results. [1] A successful message delivery indicator therefore does not establish that appropriate patient follow-up occurred.

Verify the information

AHRQ describes check-back, or repeat-back, as a way to verify exchanged information. [2] Hearing the result correctly and assigning responsibility for assessment are related but distinct tasks. A reliable local process needs both; an automated notification should not silently substitute for clinical ownership.

An ICORP implementation proposal

Have clinical teams, laboratory staff and imaging services agree which results require escalation, the primary recipient, a backup contact and the route when no one responds. Let clinical leaders set appropriate urgency categories and response expectations. Avoid applying one invented time limit to every kind of result or patient circumstance.

Follow the work through

Review unacknowledged notifications separately from acknowledged results without a documented follow-up plan. Test cover arrangements during leave, shift changes and post-discharge care. These monitoring ideas are editorial proposals. The cited Quick Safety publication is educational information and explicitly is not an accreditation standard or a new requirement. [1]
