Credentialing and privileging: qualifications are only the starting point
Distinguish source verification, authorisation of clinical scope and performance review.
Why this matters
A complete licence file does not establish which procedures a clinician should perform. In the US Joint Commission example, primary source verification confirms qualifications with the issuing source or an eligible source rather than relying only on a copy. [1] Collecting documents, verifying them and authorising a scope of work are separate activities.
Evidence and important boundaries
US Joint Commission education separates credential and competence information from clinical privileging decisions and performance monitoring. [2] Its OPPE approach uses quantitative and qualitative information to examine practice trends. [3] These are US references: their codes, intervals and requirements must not be automatically substituted for JCI, HA or Thai professional rules.
Suggested local application
ICORP suggests mapping applications, source checks, assessment of requested scope, authorised decisions and communication to clinical units. Link decisions to supporting evidence and define how requests for new activities are reviewed. Design the process against the laws, professional requirements and actual accreditation programme applicable to the hospital.
What to monitor
Local review could track verification status, pending decisions, visibility of approved scope and use of performance information in reassessment. Activity volume alone should not be the sole assessment of competence. This article distinguishes processes; it neither grants individual privileges nor certifies JCI or HA compliance.
For learning and quality improvement. This article synthesizes the references below. Practical suggestions are editorial proposals to adapt to local policies and context. It is not an official interpretation of JCI/HA requirements or guidance for treating an individual patient.
