PREMs, PROMs and satisfaction: measuring the right patient perspective

Separate care experiences, expectations and self-reported health outcomes.

Why this matters

High satisfaction does not answer every quality question. AHRQ distinguishes patient experience—what happened during care—from satisfaction, which concerns whether expectations were met. [1] Before choosing a survey, teams should decide exactly which aspect of care they need to understand.

Evidence and important boundaries

Patient-Reported Experience Measures (PREMs) examine encounters such as communication or coordination. Patient-Reported Outcome Measures (PROMs) concern self-reported health status and quality of life. They complement rather than replace one another. [2] CAHPS also distinguishes composite, single-item and overall rating measures; these are not interchangeable scores. [3]

Suggested local application

ICORP suggests starting with a focused improvement question, such as whether discharge explanations were clear. Select an appropriate instrument and language. When using a standard survey, follow its administration and scoring guidance. Keep locally added questions separate from standard scores and offer a response route that does not make patients fear an effect on their care.

What to monitor

Report respondent numbers, collection period, method and service coverage. Use comments to identify processes needing attention. Do not infer that favourable experience scores establish safety in every clinical domain, or directly compare different instruments. These are proposed reporting practices, not a JCI or HA accreditation scoring formula.
