Teach-back at discharge: checking understanding beyond “yes”

Use patients’ own explanations to check discharge communication and plan follow-up.

Why this matters

Handing over a discharge leaflet does not establish that a patient can use it. AHRQ describes teach-back as asking patients or caregivers to explain information or intended actions in their own words. It checks the clarity of the explanation, rather than testing the patient or asking for memorised repetition. [1]

Evidence and important boundaries

TeamSTEPPS includes medication instructions, follow-up testing and care plans among its applications, including communication through interpreters. [2] The RED Toolkit places assessment of understanding within discharge planning and subsequent support. [3] Teach-back therefore supports a wider transition process; it does not eliminate every discharge risk.

Suggested local application

ICORP suggests piloting one discharge pathway. Agree the essential messages, use plain language, and invite the person to explain what they will do. Re-explain unclear points and check again. Keep written instructions consistent with the discussion. The treating team must determine the clinical content; this article proposes a communication workflow, not treatment instructions.

What to monitor

For local learning, review which key topics were checked, misunderstandings found and how they were resolved. Observe conversations alongside records rather than relying only on a completed education checkbox. Use findings to improve explanations, never to blame patients. These suggested measures are not numerical JCI or HA requirements.
